Recreational Registration |
|
The OCYSA online registration program is processed on a secure server using the highest security currently available. The system allows payment of registration fees by credit card or by check. It is recommended that all registrations be paid by credit card (especially registrations completed at deadline) as registration will not be processed until payment is received. Please review the information below before starting online registration.
Regular Registration Deadline: June 30, 2008 Registrations received after deadline will be placed on a reserve list with no guarantee of player assignment. Important Note: Any registrations received and accepted after the Regular Registration Deadline will be assessed a $10.00 late fee.
Required Registration Submissions: · Payment (if paying by check) · Copy of Birth Certificate (see below) · OCYSA Medical Release Form
Players will not be assigned to a team until registration fees and birth certificates are received by OCYSA. Players will not be allowed to practice or play until an OCYSA Medical Release Form Is received by OCYSA or by the player’s coach at their first team practice.
Send checks and birth certificates to OCYSA, P.O. Box 801, Crestwood, KY 40014. Birth certificates and Medical Release forms may also be faxed to 502-241-3189.
Birth Certificates All new recreational players (or returning players that have not registered with OCYSA within the last 2 years) must provide a copy of their birth certificate. Hospital, baptismal or religious certificates are not accepted. Soccer Angel Fee Payment AssistanceOCYSA provides registration fee payment assistance to children of low income families through the Soccer Angel Fund. To request Soccer Angel fee payment assistance, send a written request to: OCYSA , P.O. Box 801, Crestwood, KY 40014 or email your request to ocysa@bellsouth.net. Parental Participation Policy OCYSA is a non-profit organization managed and operated by volunteers. Parental participation in operating the program is needed and required. If you can't volunteer, please pay a non-participation donation of $25.00 per player at registration. Please Volunteer to Coach Teams must have at least one parent volunteer to be coach or risk being disbanded. Please don't wait for the call...sign-up to coach during registration so there is time to attend OCYSA coaches training and orientation. Coaching children is a rewarding experience. Team Formation It is the goal of OCYSA to balance the ability of all recreational teams in each age group. All teams are formed by a random draw process. Little Kicks clubs are formed each season while U8 and older teams are formed for the seasonal year.
